HIPPA Form.pdf -- Notice of privacy practice summary -- this summary discloses how health information about you may be used; for treatment, to obtain payment for treatment with your authorization as required, for administrative purposes, and evaluate the quality of care that you receive.
Patient Information.pdf -- "Welcome" page -- Provides patient demographic information, emergency contact information, insurance information and office financial policy information.
Health History.pdf -- the first of the two-page form with human anatomic diagram used for the patient to specify major complaint, condition, exact location of pain, relative signs and symptoms, dates of last examinations or tests, list of habits, and medications. The second of the two-page form identifys any hospitalizations, operations, accidents or injuries along with a checklist for system-specific health information.
